Popliteal artery entrapment syndrome (PAES) is a rare but important cause of peripheral arterial insufficiency manifested by chronic lower leg pain and intermittent claudication in young athletes. 1, 2 The syndrome was first described in 1879 in a 64-year-old by a medical student, Anderson Stuart, during an anatomic dissection.

When entrapment has produced popliteal artery occlusion, positional maneuvers will not aid in the diagnosis . In our study, DSA confirmed the diagnosis in five limbs. In the other five limbs, DSA showed the occlusion of the popliteal artery, the presence of large collateral arteries, and normal proximal and distal arteries. Popliteal artery entrapment syndrome (PAES) is an uncommon cause of lower extremity claudication that is often overlooked.

The occlusion is unilateral however the anatomic anomaly is bilateral, which is often the case.

A poplitea entrapment

The popliteal artery entrapment syndrome is a rather uncommon pathology, which results in claudication and chronic leg ischemia. The popliteal artery may be compressed behind the knee, due to congenital deformity of the muscles or tendon insertions of the popliteal fossa.

We report a 54-year-old woman with PAES of total popliteal arterial occlusion was successfully treated by release of the muscle bundle and reconstruction of the popliteal artery. Popliteal artery entrapment syndrome is an uncommon entity typically affecting young athletic males who present with symptoms of calf claudication. It is caused by an anomalous relationship of muscle and artery in the popliteal fossa resulting in extrinsic arterial compression. Type II entrapment involves disruption in migration of the medial head of the gastrocnemius by a prema - turely formed popliteal artery. In Type III entrapment, abnormal remnants of mesoderm may be present, causing slips of muscle or fibrous bands to entrap the artery.
